When ordering, please indicate the following:

	Child's Name
	Preferred Type Style
	Preferred Base Style
	Color of child's room/Color to match - colors that compliment the child's room will be used. If you do not have a color palette preference, I would be happy to choose one for you.
	Theme or Accent (i.e. flower, anchor, Noah's Arc, etc...)
	Bow or No Bow - add a decorative bow to hang your name sign with (an additional $5)
